A test report confirming the diagnosis of incurable cancer is required. This can be a pathology report, CT Scan report, or other verifying test. Saint Rose's Home application must be completed and must be accompanied by a recent History and Physical from a physician. Results from tuberculin tests performed within the last six months, are also required. This may be a chest X-ray or a Mantoux Test.
